How your information is collected, used, and protected at SKYE Physiotherapy, in plain language.

1. Who we are
SKYE Physiotherapy ("SKYE," "we," "us") is a physical therapy practice owned and operated by Chang-Min (Skye) Lee, DScPT, in Columbia, Maryland. This Privacy Policy explains how we collect, use, and protect information when you visit skyephysiotherapy.com (the "Site"), contact us, or book an appointment. Health information created or received in the course of your care is protected under HIPAA and governed by our Notice of Privacy Practices, summarized in section 8.
2. Information we collect
Information you give us includes contact and inquiry details (name, phone, email, and the message you send us by email or phone); booking details collected by our scheduling provider IntakeQ, which processes this information on our behalf under a business associate agreement; and your email address if you subscribe to our newsletter. Information collected automatically includes pages viewed, time on site, referring pages, approximate location, browser and device type, and IP address, collected through cookies and similar technologies described in section 4. The public Site has no medical intake form; anything clinical is collected through IntakeQ when you book, or in the visit itself. Please do not send detailed health information by email; bring it to your appointment.
3. How we use information
We use this information to respond to your inquiries and schedule or confirm appointments; send appointment reminders and practice communications you have requested; send educational newsletters you have subscribed to, which you can unsubscribe from at any time; understand how the Site is used so we can improve it; measure and improve our advertising; and comply with legal obligations and protect the security of the Site. We do not sell your personal information.
4. Cookies analytics and advertising
The Site uses Google Analytics 4 to measure visits and how the Site is used; Google Ads conversion and remarketing tags to understand whether our advertising leads to appointment requests, which may show you SKYE ads on other websites after you visit; Wix, our website platform, which sets cookies necessary for the Site to function; IntakeQ, which sets cookies necessary for the embedded booking tool; and YouTube, which may set cookies if you play an embedded video. You can manage ad personalization at adssettings.google.com, opt out of interest-based ads at optout.aboutads.info, and control cookies through your browser settings. Blocking some cookies may affect how the booking tool and other features work.
5. How we share information
We share information only with service providers who help us operate the Site and the practice, each limited to the purposes described here; when required by law, to respond to lawful requests, or to protect our rights, clients, or staff; and in connection with a sale, merger, or transfer of the practice, subject to this policy.
6. Your choices and rights
You can unsubscribe from marketing emails using the link in any newsletter or by emailing info@skyephysiotherapy.com. For cookies and ads, see section 4. To request access to, correction of, or deletion of your information, email info@skyephysiotherapy.com. Requests about your medical record are handled under HIPAA and our Notice of Privacy Practices. The Site does not currently respond to browser Do Not Track signals.
7. Data retention and security
We keep contact and inquiry information only as long as needed for the purposes above or as required by law. Health records are retained according to Maryland law and professional requirements. We use reasonable administrative, technical, and physical safeguards to protect information, though no method of transmission over the Internet is completely secure.
8. Notice of Privacy Practices HIPAA
As a healthcare provider, SKYE Physiotherapy is a HIPAA covered entity. Protected health information about your care is used and disclosed for treatment, payment, and healthcare operations, and otherwise only with your written authorization or as permitted or required by law. You have the right to access, request amendments to, and receive an accounting of certain disclosures of your records; to request restrictions and confidential communications; and to receive a paper copy of our full Notice. You may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate against you for filing a complaint.
9. Children
The Site is not directed to children under 13, and we do not knowingly collect personal information from children online. Minors are treated with the consent and involvement of a parent or guardian.
10. Changes to this policy
We may update this policy from time to time. The effective date above shows when it was last revised. Material changes will be posted on this page.
